The information for the Restaurant Report Card comes from the Portsmouth Health Department. The results are in random order and not “ranked.” Not all restaurants are inspected every month. Health Code violations are separated into “Critical” and “Non-Critical.” Data is taken directly from the health inspector’s report without any commentary.
This report is not intended to besmirch any local business. Instead, we hope this is a learning tool for all food service workers and managers. Along with informing the public, please use this information to improve your career knowledge and abilities. As my Grandpa often said, “Don’t let the same dog bite you twice.” New mistakes are expected. Don’t get caught up in a cycle of the same mistakes.

- Observed grease build-up on the outside of all the fryers. Observed some grease on the hood over the cooking units. Observed food debris on bottom shelves of all the coolers and freezers in the facility. Observed food debris on the stove. Non-food contact surfaces of equipment are to be kept free of an accumulation of dust, dirt, food residue, and other debris.
- Repeat Observed the outside freezer unit to have an excessive build-up of ice. Observed the floor of the outside freezer unit to be loose and bent. Observed the seal on the small freezer in the kitchen to need repaired. Observed the transition piece between the carpet and wood floor between the main seating area and the bar to be broken. Physical facilities are to be maintained in good repair.
- Observed the floor of the large walk-in cooler outside to need cleaned. Facility are to be cleaned as often as necessary to keep them clean.

- Observed exit door at the back to have gaps near the sweep of the door. Outer openings are to be protected.
- Repeat Observed onions being stored in a 5-gallon bucket marked pickles. SIngle-service and single-use articles are not to be reused.
- Repeat Observed cutting board on the prep station nearest the microwave cannot be effectively cleaned and sanitized. Cutting blocks or boards are to be effectively cleaned and sanitized.
- Observed coffee grounds on the grinding machines and counter on the self-service counter at the front of the store. Observed food debris on the shelf under the dual coffee maker. Observed food debris on the shelf under the panini press. Observed coffee beans and grounds underneath the espresso machine. Observed food debris on the bottom shelves and ice chunks in the upright freezer in the back. Observed food debris on the bottom shelf of the freezer underneath the espresso machine. Observed dust on the vent of the ice machine. Observed dust on two utility fans – one sitting on top of the deli case and another on top of the ice machine. Non-food contact surfaces of equipment are to be kept free of an accumulation of dust, dirt, food residue, and other debris.
- Observed debris on the floors of the facility, especially in the hard-to-reach places like under shelving units. Physical facility is to be cleaned as often as necessary to keep it clean.

- Observed unisex employee restroom trash can to not have a lid. A toilet room used by females is to be equipped with a covered receptacle for sanitary napkins.
- Observed debris throughout the facility in the hard-to-reach areas under shelving units and other equipment. Physical facilities are to be cleaned as often as necessary to keep them clean.

The following violation(s) have been corrected since the last inspection.
- *Critical Violation* PIC unable to demonstrate knowledge of food temperatures and times for safe cooking.
- *Critical Violation* Observed the inside of the ice machine to have a blackish substance. Equipment food-contact surfaces or utensils shall be clean to sight and touch.
- Observed single use containers being used as serving utensils in the food prep cooler closest to the hand washing sink. In-use utensils shall be stored in the food with their handles above the top of the food and the container.
- Observed plastic drinking cups stacked wet. Observed pans used for queso and salsa wet stacked. Equipment and utensils shall be air dried before contact with food.
- Observed hood above the grill to have dust and debris accululation. Non-food contact surfaces of equipment shall be kept free of an accumulation of dust and other debris.
- Observed “fake” flowers and greenery attached to the walls and ceiling in the women’s and men’s bathrooms. Attachments to the wall and/or ceiling shall be easily cleanable.
These are the results of the follow-up inspection.
- Repeat Observed the cutting board on the prep cooler closest to the hand washing sink to be unclean and some deep knicks in the surface. Cutting blocks or boards shall be resurfaced if they can no longer effectively cleaned and sanitized.
- Observed a small window beside the back door to have dust accumulation on the slats and the window is open during business hours. Observed ice buildup in walk-in freezer on the walls, ceiling, floor, shelving, food, and from the unit. Facility shall be cleaned as often as necessary to keep them clean.
- Observed part of the bottom of the block wall to be missing beside the walk-in freezer. Observed seal on walk-in freezer in disrepair. Observed the fan on the left side of the condenser unit in the walk-in freezer to not be operating. Observed the hand washing sink coming loose from the wall. Physical facilities shall be maintained in good repair.

Critical Corrected During Inspection *Critical Violation* Observed sliced onions and peppers in cooler with no date mark. Observed container labeled jalapenos in cooler to not have a date mark. Refrigerated, ready-to-eat, TCS foods held for more than 24 hours are to be clearly marked to indicate the date or day by which the food is to be consumed, sold, or discarded. If taken from a manufacturer container and placed in another container, the day or date marked cannot exceed the manufacturer’s use-by date.
- Corrected During Inspection Observed the inside top of the microwave to have caked-on debris. The cavities of microwave ovens are to be cleaned at least every 24 hours.
- Observed the main door at the entrance to have pieces of the door sweep missing. Observed the door on the opposite side near the bar to be missing pieces of the door sweep. Outer openings are to be protected against the entry or insects and rodents with tight-fitting doors.
- Corrected During Inspection Observed utensils improperly stored. Clean equipment and utensils are to be stored in a self-draining position and covered or inverted.
- Observed freezers to have food debris on the bottom in all of them. Observed food debris in the corner of the pizza oven. Observed debris in the rails of the pop/beer cooler. Non-food contact surfaces of equipment are to be kept free of an accumulation of dust, dirt, food residue, and other debris.
- Observed ice build-up in the freezer reach-in unit dripping down throughout the freezer. Facility is to be cleaned as often as necessary to keep it clean.

- Observed debris on floor behind/beside the Pepsi cooler in the front of the store. Observed the vent in the ceiling by the women’s restroom, vent above the order station, vent above the stove hood, security camera, and ceiling to have dust and debris build-up. Observed the wall behind the ovens to have dust and debris build-up. Facility shall be cleaned as often as necessary to keep them clean.
