Portsmouth Ohio Restaurant Report Card

The information for the Restaurant Report Card comes from the Portsmouth Health Department. The results are in random order and not “ranked.” Not all restaurants are inspected every month. Health Code violations are separated into “Critical” and “Non-Critical.” Data is taken directly from the health inspector’s report without any commentary.

This report is not intended to besmirch any local business. Instead, we hope this is a learning tool for all food service workers and managers. Along with informing the public, please use this information to improve your career knowledge and abilities. As my Grandpa often said, “Don’t let the same dog bite you twice.” New mistakes are expected. Don’t get caught up in a cycle of the same mistakes.

PIC unable to demonstrate knowledge by having no critical violations. PIC also unable to demonstrate knowledge of general practices within the facility. Correct immediately.

3717-1-03.1(A)(1)/Sources – compliance with food law.

*Critical Violation* Observed a jar of “Fire Cider” on the shelf with the drink syrups in the prep room and bottles of Tinctures for sale in the front area of the establishment. Foods are to be obtained from sources that comply with law. Correct immediately.

3717-1-07.1(M)/Medicines – restrictions and storage.

Observed throat spray (personal use) to be stored on a shelf in the prep room with the drink syrups. Medicines that are for the employees’ use are to be labeled and located to prevent the contamination of food, equipment, utensils, linens, single-service/use articles. Correct by next inspection.

3717-1-07(B)/Poisonous or toxic materials: Working containers – common name.

*Critical Violation* Observed a spray bottle with blue liquid in it on a shelf in the laundry room to not be labeled. Working containers of poisonous or toxic materials such as cleaners and sanitizers taken from bulk supplies are to be clearly and individually identified with the common name of the material. Correct immediately.

*Critical Violation* Observed ants on the second shelf with the syrups in the prep room. Observed the presence of flies and gnats in the prep room. Observed a live ant in the laundry room. The presence of live insects and other pests.is to be controlled to minimize their presence on the premises. Correct immediately.

3717-1-03.2(M)/Wiping cloths – use limitation.

Observed barista wiping the counter with a cloth that had not been stored in sanitizing solution. Cloths that are in use for wiping counters and other equipment surfaces are to be held between uses in a chemical sanitizer solution. Correct by next inspection.

3717-1-03.2(K)/In-use utensils – between-use storage.

Observed the ice scoop in its entirety to be stored in the ice. In-use food dispensing utensils are to be stored in a clean, protected location (ice scoops used to non-TCS food). Correct by next inspection.

3717-1-04.4(B)/Cutting surfaces.

Observed yellow cutting sheet to be severely scored and stained. Cutting blocks or boards are to be discarded if they cannot be effectively cleaned and sanitized. Correct by next inspection.

3717-1-04.5(A)(3)/Cleanliness of nonfood-contact surfaces of equipment.

Observed excessive coffee grounds on the counter and grinding machines. Observed spillage on the grate and backsplash of the espresso machine. Non-food contact surfaces of equipment are to be kept free of an accumulation of dust, dirt, food residue, and other debris. Correct by next inspection.

Observed the spigot in the hallway to be leaking. Plumbing system is to be properly maintained or repaired. Correct by next inspection.

3717-1-05.4(K)/Storing refuse, recyclables, and returnables.

Observed several trash bags with refuse and/or possibly recyclable materials to be sitting around the facility (behind the counter, in the prep room, and in the room between the restroom and laundry room). Refuse or recyclables are to be stored in receptacles so they are inaccessible to insects and/or rodents. Correct by next inspection.

3717-1-06.4(F)/Drying mops.

Observed mop to be laying in the mop sink soaking wet. After use, mops are to be placed in a position that allows them to air-dry without soiling walls, equipment, or supplies. Correct by next inspection.

3717-1-06.1(C)/Floor and wall junctures – covered, and enclosed or sealed.

Observed the plastic trim in the restroom does not properly connect to the floor causing a gap that allows for water to accumulated behind the trim. Floor and wall junctures are to be properly covered or closed and/or sealed. Correct by next inspection.

3717-1-06.4(B)/Cleaning – frequency and restrictions.

Observed large amount of coffee grounds on the floor by the grinding machines. Facility is to be cleaned as often as necessary to keep it clean. Correct by next inspection.

The follow up inspection:

  • Repeat Observed plywood being used as shelving for drinkware. Nonfood-contact surfaces are to be constructed of corrosion-resistant, nonabsorbent, and smooth material.
  • Observed test strips to be out-of-date. A test kit or other testing device that accurately measures the concentration in ppm of sanitizing solution is to be provided. Correct by next inspection. **Test strips have been ordered but not received.**
  • Observed the wall behind the mop sink to not be properly sealed. **The drywall has been covered with cleanable surface but has not been sealed.** Walls are to be finished and sealed to provide a smooth, nonabsorbent, easily cleanable surface.

*Critical Violation* Observed blackened debris inside the ice machine. Observed build-up of food residue on the egg griddle (PIC states that it is cleaned every day around 2:00 p.m. but had not had a chance to get to it today). Equipment food-contact surfaces or utensils are to be clean to sight and touch. Correct immediately.

3717-1-03.4(H)(1)/Ready-to-eat, time/temperature controlled for safety food – disposition.

*Critical Violation* Observed scrambled egg seasoning with a disposition date of 4/24/25. Observed Iced Cappuccino Mix with a disposition date of 5/4/25. Observed two cartons of mixed berry fruit smoothie mix with disposition dates of 4/10/25. Ready-to-eat, TCS food is to be properly discarded when required. Correct immediately.

3717-1-04.8(A)/Equipment and utensils – air-drying required.

Observed a plastic bottle on the drying rack labeled scrambled eggs is upright and capped with a lid while the inside of the bottle is wet. Equipment and utensils are to be air dried or adequately drained before contact with food. Correct by next inspection.

3717-1-04.5(A)(3)/Cleanliness of nonfood-contact surfaces of equipment.

Observed the small black Lasko floor fan to have dust and residue on it. Observed the gasket on the cooler storing raw eggs has egg residue in it. Observed food residue caked on the inside of both ovens. Observed the inside bottom of the donut display case to have an accumulation of sprinkles and icing debris. Observed the gaskets inside of all the creamer machines to have food residue caked inside of them. Observed food residue on the back inside ledge of the food prep station containing sandwich toppings and cream cheese spreads. Non-food contact surfaces of equipment are to be kept free of an accumulation of dust, dirt, food residue, and other debris. Correct by next inspection.

3717-1-06.4(B)/Cleaning – frequency and restrictions.

Observed pop syrup residue on the wall behind the rack holding active syrups. Observed a black substance along the caulking on the edge along the mop sink. Observed the corners and areas under all the shelving and equipment to have accumulated debris under them. Observed debris on the floors under the shelving in the walk-in cooler and freezers. Facility is to be cleaned as often as necessary to keep it clean. Correct by next inspection.

Observed a wet mop sitting in the mop sink. Mops are to be placed in a position that allows them to air-dry without soiling walls, equipment, or supplies. Correct by next inspection.

3717-1-06.4(D)/Cleaning ventilation systems, nuisance, and discharge.

Observed the vent above the warewashing sink to have black residue and dust on it and the ceiling around it. Ventilation systems are to be cleaned and filters changed so they are not a source of contamination by dust, dirt, and other materials.

Critical Repeat *Critical Violation* Observed numerous rodent bait boxes located within the facility. The presence of live insects, rodents, and other pests is to be controlled to minimize their presence on the premises. Records of rodent control were requested and provided to the inspector.

Observed the gasket on the small cooler containing raw eggs to be in disrepair. Equipment components are to be kept intact, tight or properly adjusted. Correct by next standard inspection.

3717-1-06.4(A) / Repairing.

Observed the floor in the dry goods storage area in front of the mop sink to be severely cracked allowing water to stand. Observed broken tile along the walls of the walk-in cooler to be broken and in disrepair. Observed cracked tiles in the kitchen in front of the ice machine and close to the egg station. Physical facilities are to be maintained in good repair.

Observed employee drinks and food intermingled with food and utensils used for the FSO. Employees eating, drinking, or using tobacco products in designated areas where contamination cannot result. Correct by next inspection.

Observed a ziploc bag with a white powder substance in it to not be labeled. Working food containers are to be properly labeled with the common name of the food. Correct by next inspection.

3717-1-02.3(D)/Hair restraints – effectiveness.

Observed employees preparing food with beards and long hair hanging loose. Food employee(s) are to effectively restrain hair by wearing hair restraints that are designed and worn to effectively keep their hair from contacting exposed food, clean equipment/utensils, or unwrapped single-use or single-service articles. Correct by next inspection.

Observed dust accumulation in the exhaust fans of both the men’s and women’s restrooms. Facility is to be cleaned as often as necessary to keep it clean.

Repeat Observed food residue in the bottom of the small freezer unit containing breaded foods. Non-food contact surfaces of equipment are to be kept free of an accumulation of dust, dirt, food residue, and other debris. 

Repeat Observed that the FSO did not submit facility layout and equipment specifications for modifications made to the FSO to add a kitchen space. Correct immediately by obtaining the proper permits and certifications from local authorities. **PIC and owner have began plan review application process.**
