The information for the Restaurant Report Card comes from the Portsmouth Health Department. The results are in random order and not “ranked”. Not all restaurants are inspected every month. These inspections occurred within the last 30 days. Health Code violations are separated by “Critical” and “Non-Critical”.

This report is not intended to besmirch any local business. Rather, we hope this is a learning tool for all food service workers and managers. Along with informing the public, please use this information to improve your knowledge and abilities in your careers. As my Grandpa said, many times, “Don’t let the same dog bite you twice”. New mistakes are expected. Don’t get caught up in a cycle of the same mistakes.

Corrective Action: The manager said she would talk to the trainee who stored the pans improperly about storing them inverted or covered in the future. Correct by next inspection.

Corrected During Inspection Observed egg spill on the floor that appeared to have been there awhile behind the McFlurry counter. Corrective Action: The manager had an employee clean up the spill.

Critical Corrected During Inspection Observed raw eggs being stored above Ready-to-eat products in the prep cooler across from the fry grill area. Corrective Action: The employee moved the eggs to the bottom shelf.

Observed multiple food items being stored uncovered in the cooler units. Corrective Action: To prevent contamination food products should be stored covered. Correct by next inspection.

Observed facility without sanitizing test strips. Corrective Action: Person-in-charge said he hasn’t been able to get any in and will look on Amazon this afternoon. Correct by next inspection.

Observed ice build-up in the reach-in Frigidaire freezer. Observed dust and debris build-up on the canned goods rack by the back door. Observed racks of the Victory cooler with a build-up of debris. Observed bottom of prep coolers with a build-up of debris. Observed fry grill hood with a build-up of oil. Corrective Action: All nonfood-contact surfaces should be cleaned more frequently to prevent the build-up of debris. Correct by next inspection.

Observed floor tiles in front of the fry grill broken. Observed the wall paint chipping away on the wall in the storage room. Corrective Action: Ensure that the facility is kept in good repair. Correct by next inspection.

Corrected During Inspection Observed frozen shrimp being thawed improperly in the prep sink. Corrective Action: An employee started running water over the shrimp to thaw it properly.

Observed clean dishes not being stored inverted on the top shelf across from the meat prep area. Corrective Action: Store all clean equipment and utensils either covered or inverted to prevent contamination. Correct by next inspection.

Observed food debris build-up on the bottom shelf of multiple prep units and coolers throughout the facility. Corrective Action: Clean the bottom of the prep units and coolers out frequently to prevent debris build-up. Correct by next inspection.

Observed debris build-up on the floors underneath equipment in the kitchen area. Corrective Action: Clean these spaces more frequently to prevent debris build-up. Correct by next inspection.

Repeat Observed multiple floor tiles broken or missing in the kitchen area. Corrective Action: Keep the facility maintained in good repair. Correct by next inspection.

Critical **Critical Violation** Observed ice machines bins with black debris build-up in the kitchen, catering, and the cafe. Observed the ice shaft unclean in the Pepsi machine. Observed grinder that needs to be cleaned in the allergen area. Observed unclean nozzles in the flavored water machines. Corrective Action: The manager closed the ice machine in the catering area.

Critical **Critical Violation** Observed cooked brussel sprouts being held at 100.6 degrees. Corrective Action: Brussel sprouts were removed from service.

Critical Corrected During Inspection **Critical Violation** Observed roast beef labeled as being made on 11/8/21 in the walk-in cooler. Corrective Action: The manager threw the roast beef out.

Critical **Critical Violation** Observed unlabeled chemical bottles by the chemical storage rack.

Observed facility does not have an employee with manager certification in food protection. Correct by next standard inspection.

Observed no written procedures for responding to vomiting or diarrheal events. Corrective Action: A copy of the ODH vomiting and diarrheal clean-up procedure was given to the manager.

Handwashing sink in catering room not reaching 100 degrees.

Observed blackened debris build-up in the Waring oven.

Observed salad buffet not maintaining temperatures at the capacity needed.

Observed multiple unlabeled food storage containers throughout the facility (containing pepper, oil, flour, cake mix, etc.).

Observed multiple dead gnats in the cold storage case in the cafe.

Observed multiple dishes throughout the facility that have not been allowed to properly air dry.

Observed wiping cloths being stored underneath the waffle maker in the cabinet.

Observed multiple single-use spice containers being reused for different food items.

Corrected During Inspection Observed absorbent material being used to put oil on the grill.

Observed cutting boards throughout the facility that cannot be effectively cleaned and sanitized and need to be replaced or resurfaced.

Observed a noncommercial fridge/freezer in the allergen area of the kitchen. Observed a noncommercial grade toaster and crockpot in the catering area. Correct by next standard inspection.

Observed stickers left on pans after washing in the hot entrees area of the kitchen.

Observed cabinets underneath the soda machines with a build-up of debris. Observed the compartments with missing drawers missing in the kitchen with a build-up of debris.

Observed drainage issue in the hand sink in the hot entrees section of the kitchen.

Corrected During Inspection Observed mop being stored improperly in the cafe.

Observed silverware tray broken in the catering area. Observed seal on the Traulsen freezer by the french fry grill in disrepair. Observed soda nozzles are worn down and in need of replacing.

Observed plastic debris in the light fixtures throughout the kitchen. Observed dust build-up on the ceiling of the walk-in freezer.

Critical *Critical Violation* Observed spray bottle with a clear substance on storage rack next to the three-compartment sink with other cleaner and sanitizers without a label.

Critical Repeat **Critical Violation** Observed wastewater surfacing in the mop sink next to the exit door. Corrective Action: Repair the plumbing issue and clean the affected area.

Repeat Observed grease build-up on walls and floors throughout the facility. Observed debris build up in corners and against walls of the facility. Corrective Action: The facility should be cleaned more frequently to prevent debris build-up. Correct by the next standard inspection.

Repeat Observed leaking hand sink near the grill area. Corrective Action: Ensure that facility is kept in good repair. Correct by next standard inspection.

Corrective Action: The manager has new vents for the hood he said he would install and said he would get someone in to look at the walk-in to determine why it has the ice build-up. Correct by next inspection.
